The questionnaire is filled out for each tourist, regardless of age, at the link: https://imigresen-online.imi.gov.my/mdac/main?registerMain

The questionnaire is filled out no earlier than 3 days before the start of the tour.

If the tour is combined and includes travel to neighboring countries, a questionnaire must be filled out for each entry into Malaysia.

STEP 1. Entering all data

The questionnaire is divided into 2 parts.:

1. Personal information about the tourist
2. Travel information

Example of filling:

• Name – Last name and first name as in the passport
• Passport No. – passport number
• Date of Birth – date of birth
• Date of Passport Expiry – passport expiration date
• Nationality / Citizenship – select citizenship from the list provided
• Sex – gender, male MALE or female FEMALE
• Email Address – a current address is required to receive a personal code when confirming the completion of the migration form
• Confirm Email Address – entered manually, previously entered address
• Country / Region Code – international telephone code, for Georgia
• Confirm Country / Region Code – duplicates the previous field
• Mobile Number – indicates a current mobile phone number
• Confirm Country / Region Code – duplicates the previous field

• Date of Arrival – date of arrival in Malaysia, selected from the calendar – possible dates are shown in black
• Date of Departure – date of return flight from Malaysia
• Mode of Travel – mode of travel, selected from the suggested ones (air – plane, sea – sea, land – land passage)
• Last Port of Embarkation – for departures from Georgia, select from the list Georgia
• Accommodation of Stay – accommodation for stay, selected from the suggested ones, if you have a hotel booked – you must select HOTEL
• Address (In Malaysia) – address of temporary stay in Malaysia, it is enough to indicate the name of the hotel
• State and City – state, selected from the list of suggested ones, main options:

• Postcode – indicated in the hotel address, 5 digits, can be checked on the website https://postcode.my/location/

STEP 2. Data verification

After entering all the data, click the SUBMIT button

A notification about the successful completion of the questionnaire appears

STEP 3. Getting a PIN

A confirmation is sent to the specified email address, which tourists must save and provide upon arrival when going through passport control at the airport.
